লোকো ইয়ার্ডে দুর্ঘটনা : ১৪০৯:
(a) In these cases action shall not be initiated by the station master. but by the Loco Foreman or other subordinate concerned.
(ক) লোকো ইয়ার্ডে দুর্ঘটনার ক্ষেত্রে স্টেশন মাস্টার কোনো পদক্ষেপ শুরু করবেন না, বরং লোকো ফোরম্যান বা সংশ্লিষ্ট অন্যান্য অধস্তন কর্মকর্তা প্রয়োজনীয় ব্যবস্থা গ্রহণ করবেন।
(b)In accidents resulting in loss of life or grievous hurt the following should be advised
(খ) যেসব দুর্ঘটনায় প্রাণহানি বা গুরুতর জখম হওয়ার ঘটনা ঘটে, সে ক্ষেত্রে নিম্নলিখিত ব্যক্তিদের অবহিত করতে হবে:

১. চিফ পার্সোনেল অফিসার (সিপিও) ২. সংশ্লিষ্ট বিভাগীয় প্রধান ৩. ডিস্ট্রিক্ট ম্যাজিস্ট্রেট (জেলা প্রশাসক) ৪. সরকারি রেলওয়ে পরিদর্শক (জিআইআর) ৫. স্থানীয় থানার ভারপ্রাপ্ত কর্মকর্তা (ওসি)

(গ) অন্যান্য দুর্ঘটনার ক্ষেত্রে, সংশ্লিষ্ট অধস্তন কর্মকর্তা কেবল একটি বার্তার মাধ্যমে তার জেলা কর্মকর্তাকে অবহিত করবেন এবং/অথবা যদি তার কোনো জরুরি সহায়তার প্রয়োজন হয়, তবে তিনি যার কাছ থেকে সহায়তা প্রয়োজন তাকে টেলিগ্রাম করবেন।

(ঘ) জেলা কর্মকর্তাকে অবহিত করার পর 'EB_G 38' ফরমে একটি বিস্তারিত প্রতিবেদন জমা দিতে হবে। এই প্রতিবেদন পাওয়ার পর, মৃত্যু বা গুরুতর আঘাতের ঘটনার ক্ষেত্রে জেলা কর্মকর্তা মেডিকেল ও পুলিশ রিপোর্ট সংগ্রহ করবেন, প্রয়োজনীয় তদন্তের ব্যবস্থা করবেন এবং ১০ দিনের মধ্যে তার বিভাগীয় প্রধানের মাধ্যমে 'EG-G 33' ফরমে চেয়ারম্যানের কাছে একটি প্রতিবেদন জমা দেবেন। এরপর প্রয়োজন অনুযায়ী ২১ দিনের মধ্যে বিভাগীয় তদন্তের বিবরণী পেশ করতে হবে। অন্যান্য দুর্ঘটনার ক্ষেত্রে, তিনি প্রয়োজনীয় তদন্ত পরিচালনা বা তার ব্যবস্থা করবেন এবং দুর্ঘটনার কারণ নিশ্চিত করে প্রয়োজনীয় পদক্ষেপ গ্রহণ করবেন। এরপর ঘটনাটি 'EG-G 330' ফরমে মাসিক বিবরণীতে অন্তর্ভুক্ত করবেন।
(c) In other accident, the subordinate concerned will merely advise his district officer by a message and/or , if he requires any immediate help will telegraph to the person from whom it is required.
(d) The advice to the District Officers should be followed up by a detailed report in Form EB_G 38 . On receipt of this report the District officer shall. in cases of the death or grievous hurt obtained Medical and police report , hold or arrange for such enquiry as may be necessary and submit a report to the Chairman through the Head of his Department in Form EG-G 33 within 10 days, followed by the proceeding of a departmental enquiry within 21 days as the case may require, In other cases he shall hold or arrange for such enquiry as may by necessary and having ascertained the cause and take necessary action, will incorporate the case in the monthly return in Form EG -G 330.
