(ক) তদন্তের মূল উদ্দেশ্য হলো দুর্ঘটনার কারণগুলো পুঙ্খানুপুঙ্খভাবে উদ্ঘাটন করা, যাতে পরবর্তীতে এই ধরনের ঘটনার পুনরাবৃত্তি রোধ করা যায়। কোনো নির্দিষ্ট ঘটনায় কেবল একজন ব্যক্তির ভুলের কারণে কিছু ঘটেছে—এটুকু খুঁজে বের করাই যথেষ্ট নয়, এর চেয়ে বেশি কিছু প্রয়োজন। তদন্তের পরিধি আরও বিস্তৃত করতে হবে যাতে এটি নিশ্চিত করা যায় যে কোনো সাধারণ শিথিলতা বা অন্য কোনো পরোক্ষ কারণ দুর্ঘটনার পেছনে কাজ করেছে কি না। অনেক ক্ষেত্রে দুর্ঘটনার পেছনে তদারকি বা সুপারভিশনের অভাব থাকে, যা কমিটির তদন্তে উঠে আসে না। কমিটি যদি মনে করে যে অপর্যাপ্ত তদারকির কারণে দুর্ঘটনা ঘটেছে, তবে তাদের সুপারিশে এই বিষয়টি স্পষ্টভাবে উল্লেখ করতে হবে।
(a) The main object of an inquiry is to ascertain fully causes of accidents with a view to preventing their recurrence. . It is not sufficient merely to find that on a particular occasion something happen through the fault of one particular man, more than this is required. The scope of the inquiry shall be extended so as to embrace a wider field and ascertain whether there has been any general laxity of working or other indirect cause or causes which have contributed to the particular accident investigated . Accidents in many causes , are not brought home sufficiently to the supper vising staff and committees, if they consider that accidents that accidents have been caused by inadequate supervision, should record their opinion and bear this aspect of the matter in mind in making their recommendations.
(খ) তদন্ত কমিটি জেলা, সহকারী বা অধস্তন কর্মকর্তাদের সমন্বয়ে গঠিত হবে। তবে ব্যতিক্রমী ক্ষেত্রে, চেয়ারম্যানের নির্দেশ অনুযায়ী তিনটি প্রধান বিভাগ—পরিবহন (Transportation), মেকানিক্যাল (Mechanical) এবং ইঞ্জিনিয়ারিং (Engineering)-এর বিভাগীয় প্রধান বা তাদের ডেপুটি এবং সংশ্লিষ্টতা থাকলে অন্যান্য বিভাগের (মেডিকেল, সিগন্যাল ও স্টোর) কর্মকর্তা বা অধস্তনদের নিয়ে কমিটি গঠিত হতে পারে।
(b) Inquiry Committees shall be constituted of District and assistant or subordinate officers. or in the exceptional causes, when ordered by the Chairman, of Heads of Departments or Duputies of the three main departments Transportation, Mechanical and Engineering , with offers or subordinates of other departments (Medical , signal and stores ) if they are concerned.
কমিটির সবচেয়ে সিনিয়র সদস্য সভাপতি হিসেবে দায়িত্ব পালন করবেন।
The most senior member of the committee shall act as president.
(গ) তদন্তে উপস্থিত থাকার জন্য নোটিশ পাওয়ার পরও যদি কোনো কর্মকর্তা বা অধস্তন কর্মচারী উপস্থিত হতে ব্যর্থ হন, তবে তাকে ছাড়াই তদন্ত কার্যক্রম এগিয়ে যাবে (হোম লাইনের কর্মকর্তা বা অধস্তনদের ক্ষেত্রে) এবং তার লিখিত ব্যাখ্যা তদন্তের নথিতে অন্তর্ভুক্ত করা হবে।
(c) If any officer or subordinate after having been notified to attend an inquiry fails to attend , the inquiry should proceed without him (for home line officer or subordinate)and his written explanation shall be incorporated in the proceedings.
(ঘ) যৌথ তদন্তের ক্ষেত্রসমূহ:

যৌথ তদন্ত (Joint Inquiry) নিম্নলিখিত ক্ষেত্রগুলোতে অনুষ্ঠিত হবে: (১) এ (১)-এর অধীনস্থ সমস্ত ক্ষেত্রে (অনধিকার প্রবেশকারীদের মৃত্যু বা গুরুতর জখম হওয়া ছাড়া) এবং এ (২) ও (৩)-এর অধীনস্থ সমস্ত ক্ষেত্রে। (২) যেকোনো শ্রেণীর দুর্ঘটনার ক্ষেত্রে, যেখানে দুর্ঘটনার কারণ স্পষ্ট নয় এবং একাধিক বিভাগ জড়িত রয়েছে। (৩) যেসব দুর্ঘটনায় কেবল একটি বিভাগ জড়িত, কিন্তু দুর্ঘটনার গুরুত্ব বিবেচনা করে সংশ্লিষ্ট বিভাগ যৌথ তদন্তের প্রয়োজন মনে করে।
Jo joint inquiries shall be held in (1) All cases under A (1) (except death or grievous injury to trespassers) and all cases under A (2) and (3)
(ঙ) সমস্ত যৌথ তদন্তের ডাক দেবে পরিবহন বিভাগ—তা নিজস্ব উদ্যোগে হোক, চেয়ারম্যানের নির্দেশে হোক বা অন্য কোনোভাবে। দুর্ঘটনার গুরুত্ব অনুযায়ী পরিবহন বিভাগ জেলা, সহকারী বা অধস্তন কর্মকর্তাদের তদন্ত আহ্বান করবে।
(2) All cases of whatever class, where the cause is not clear and more than one department is concerned.
(চ) যৌথ তদন্তে নিয়োজিত কর্মকর্তাদের তাদের তদন্তের ফলাফলে সম্পূর্ণ নিরপেক্ষ হতে হবে। মতবিরোধের ক্ষেত্রে, যেকোনো ভিন্নমত (Note of Dissent) তদন্তের ফলাভলের নিচে নথিবদ্ধ করতে হবে এবং সংশ্লিষ্ট সদস্যদের স্বাক্ষর থাকতে হবে। বিভাগীয় পক্ষপাতিত্ব এড়িয়ে চলতে হবে; কারণ তদন্তের মূল উদ্দেশ্য হলো প্রকৃত তথ্য নথিবদ্ধ করা এবং দুর্ঘটনার আসল কারণ খুঁজে বের করা, কারো দোষ আড়াল করা নয়।
(3) Accidents in which only one department is involved if in view of the importance of the accident the department concerned wishes a joint injury
(ছ) প্রাথমিক, বিভাগীয় বা যৌথ তদন্তে কোনো মিথ্যা সাক্ষ্য দেওয়া হলে সংশ্লিষ্ট কর্মচারীকে দৃষ্টান্তমূলক শাস্তি দেওয়া হবে এবং তাকে চাকরিচ্যুতও করা হতে পারে। কর্মচারীদের এটিও মনে রাখা উচিত যে, দুর্ঘটনার জন্য দায়ী কোনো ব্যক্তি যদি সততার সাথে এবং সরাসরি সাক্ষ্য দেন, তবে মিথ্যা বিবৃতির মাধ্যমে নিজের দায় এড়ানোর চেষ্টা করা ব্যক্তির চেয়ে তিনি অনেক বেশি সহানুভূতিশীল আচরণ পাওয়ার যোগ্য হন।
(c) All joint inquiries shall be convened by the Transportation Department whether on its own initiative, or under chairman's orders or not laid down by the chairman, the Transportation Department shall convene District, Assistant or Subordinate Officers inquiries according to the importance of the accident.
(জ) লোকোমোটিভের দুলুনি বা কম্পনের (Oscillation) কারণে সৃষ্ট যেকোনো অস্বাভাবিক ঘটনার বিবরণ প্রতি ত্রৈমাসিকে রেলওয়ে বোর্ডে রিপোর্ট করতে হবে।
(f) Officials on joint inquiries must be meticulously impartial in their findings. In case of disagreement, any notes of dissent shall be recorded below the findings and signed by the members concerned. Department feeling should be avoided the object being to record facts, ascertain the true cause of the accident and not to screen faults.
(ঝ) এছাড়া রক্ষণাবেক্ষণের স্বার্থে, ইঞ্জিনের যেকোনো ধরনের অস্বাভাবিক দুলুনি, ঝাঁকুনি বা অমসৃণ চলাচলের (Rough Riding) ঘটনা অবিলম্বে রিপোর্ট করতে হবে, যাতে সংশ্লিষ্ট ইঞ্জিন এবং রেললাইন তাৎক্ষণিকভাবে পরীক্ষা করা যায়। এর ফলে অমসৃণ চলাচলের জন্য দায়ী যেকোনো ত্রুটি অবিলম্বে দূর করা সম্ভব হবে।
(g) If false evidence is given at any preliminary , department or joint inquiry, exemplary punishment will be given to the staff who will be liable to the discharged. The staff should also remember that a man who is responsible for an accident, but who gives his evidence in an honest and straightforward manner is more likely to receive lenient treatment than a man who endeavours to evade his responsibility by making a false statement.
যেকোনো কারণে ইঞ্জিনের অমসৃণ চলাচলের বিষয়ে চালকদের (ড্রাইভারদের) রিপোর্ট সংশ্লিষ্ট পার্মানেন্ট ওয়ে ইন্সপেক্টর (PWI) বা সহকারী পার্মানেন্ট ওয়ে ইন্সপেক্টরকে লাইনের ত্রুটিপূর্ণ স্থানগুলো দ্রুত চিহ্নিত করতে এবং প্রয়োজনীয় ব্যবস্থা নিতে সহায়তা করবে।
(h) Cases of abnormal occurrences attributable to oscillations of loco motive are required to be reported to the Railway Board, quarterly.
চালককে অত্যন্ত সতর্কতার সাথে অমসৃণ চলাচলের স্থানটির মাইলেজ (অবস্থান) নোট করতে হবে এবং পরবর্তী স্টেশনের স্টেশন মাস্টারের কাছে মাইলেজ, ঝাঁকুনির প্রকৃতি এবং তার ইঞ্জিনের নম্বর রিপোর্ট করতে হবে। এছাড়া যাত্রা শেষে তিনি রানিং শেড ফোরম্যানের কাছে রিপোর্ট করবেন, যিনি প্রয়োজনে ইঞ্জিনটি পরীক্ষা করে ডিস্ট্রিক্ট মেকানিক্যাল ইঞ্জিনিয়ারের কাছে প্রতিবেদন জমা দেবেন। ডিস্ট্রিক্ট মেকানিক্যাল ইঞ্জিনিয়ার তার মন্তব্যসহ (যদি থাকে) প্রতিবেদনটি দুই কপি তৈরি করে চিফ মেকানিক্যাল ইঞ্জিনিয়ারের কাছে পাঠাবেন।
(i) Further in the interest of maintenance, all cause of hunting ,lurching ,swaying, or rough riding of engines should be reported to permit of immediate examination of the engine concerned and of the track, so that any fault discovered in either or both , which may have contributed to rough riding can be eliminated without delay.
চালকের কাছ থেকে এমন রিপোর্ট পাওয়ার পর, স্টেশন মাস্টারকে অবশ্যই রিপোর্টের বিবরণ, সংশ্লিষ্ট ট্রেনের নম্বর এবং ঠিক তার আগে ওই স্থান দিয়ে অতিক্রম করা ট্রেনের নম্বরসহ সংশ্লিষ্ট পার্মানেন্ট ওয়ে ইন্সপেক্টরকে তারবার্তা পাঠাতে হবে। এর অনুলিপি অ্যাসিস্ট্যান্ট ইঞ্জিনিয়ার, এক্সিকিউটিভ ইঞ্জিনিয়ার, লোকো ফোরম্যান এবং গন্তব্য স্টেশনের ডিস্ট্রিক্ট মেকানিক্যাল ইঞ্জিনিয়ারের কাছে পাঠাতে হবে। পুলড ইঞ্জিনের (Pooled Engine) ক্ষেত্রে ফিরতি যাত্রার সময় হোম স্টেশনের ডিস্ট্রিক্ট মেকানিক্যাল ইঞ্জিনিয়ারের কাছেও অনুলিপি পাঠাতে হবে।
The reports from the Drivers, of any rough riding form whatever cause will assist the permanent wy inspector or Assistant permanent way inspector in charge concerned by bringing to his notice at once places in the track which need his attention.
